To submit a request to have your placement test scores sent to another college or institution, please fill out the request form on this page. Be sure to include your Full Name, Birthdate, Student ID number (M# or SS#), and also include the date and the year that you took the placement test. Your scores will be faxed and/or mailed to the address on the request form within 3 business days.
