How to sign up for the program
Step 1: Review the Eligibility Form to determine whether or not you qualify to be a part of the LPP program.

Step 2: One of the following people should complete the Referral Form on the student’s behalf.
- Counselor
- Parent/Guardian
- Teacher
- Probation Officer
- Case Manager
- Social Worker
- School Administrator
- Community Outreach

Step 3: Return Referral Form to the LPP office at Monroe Community College.
