Note: THIS RESPONSE FORM IS ONLY FOR STUDENTS WHO HAVE RECEIVED NOTICE FROM THE GRADUATION OFFICE REGARDING PARTICIPATION IN THE CEREMONY. Please do not complete this if you are finishing your degree in Summer 2009; summer graduates must register through Early Attendance procedures.
