Document Delivery of Articles Request Form


This form is to be used by Distance Learning Students who are exclusively off campus through enrollment in only SUNY Learning Network (SLN) courses and would like to request copies of journal, magazine and newspaper articles available from our print and microfilm collections.

Articles requested will be delivered to students through their MCC Student E-mail Account.

BEFORE YOU SUBMIT A REQUEST:

  • Please search our Library Databases to see if the article you are interested is already available full-text online. To do this from your home computer, go to the Find Articles and Other Information web page and follow the instructions for logging on to the EZ Proxy server in the "Access from Home" section.
  • If the article you are looking for is not available full-text through any of the Library's online databases, search the MCC Library Catalog to make sure we own in our print or microfilm collection the title of the magazine, journal, or newspaper the article appears in.

  • A separate request must be submitted for each article.
  • Fill in all applicable fields. Press the "Tab" key to advance through the fields.
  • All fields marked with an asterisk * are required fields, and must be completed.
  • When finished, click on the "Submit Form" button at the bottom of the page.
