Documentation

Students
requesting accommodations, academic adjustments, and/or auxiliary
aids must provide written documentation
to the Services for Student with Disabilities office on the Brighton Campus or to the
Student Services Center on the Damon City Campus. Services
can not be provided for a student who does not provide the necessary documentation.
All requests for accommodations should be made as early as possible, at least 30 days in advance of the need.
Documentation must be written by a licensed or credentialed examiner; which includes a diagnostic statement identifying the disability; a description of the diagnostic methodology; a description of the current functional limitations; a description of the expected duration and progression of the condition; and a rationale and justification for all requested accommodations. Please note that submission of only an IEP or 504 Plan may not be sufficient.
Any and all information received by the college regarding an individual's disability is strictly confidential.

accommodations are not clearly identified in a diagnostic report the
student will be asked to seek clarification and, if necessary, more
information. The final determination for approving appropriate and
reasonable accommodations rests with the institution. |
