for Student Athletes
Planning on being an MCC athlete? Here are a few things you need to know prior to participation on an athletic team at MCC:
The Junior College Athletic Association (JCAA) requires that all first year athletes submit a completed Health History and Exam Form signed by their primary care provider. This form needs to be completed after July 1st of the upcoming school year and turned in prior to the start of practice.
Returning Athletes (2nd year) are required to complete a Returning Athlete's Medical History Questionnaire and have a signed clearance from their primary care provider. This form needs to be completed after July 1st of the upcoming school year and turned in prior to the start of practice.
In addition to the Immunization Requirements of all students in the general student population, athletes must also have:
- PPD (Tuberculin skin test) placement and reading within the past five (5) years
- Tetanus shot within the last ten (10) years
Student Athletes are also required to have a comprehensive insurance plan while participating on an athletic team. Please refer to our information on Athletic Injury Insurance for more information. If you do not have your own insurance, you may purchase insurance from MCC for illness and/or injury. Please go to our Insurance page for complete information.
