Students may request
a transfer of their Immunization record from Health services by a written
request, faxed request or a telephone request. Students may request this
form or come to Health Services. You may download the form by computer from
the Health Services Web Page. |

about my immunizations. Where do I get this information? |

You may call or
come to Health Services for this information. If you are registered for
the current semester, you may obtain your immunization status by calling
(585) 292-2018. Press 2 and follow the prompts. This information is updated
daily. For Fax requests at (585) 292-3856, a written release is needed. |

What Immunizations
are required for students in Health Career Programs? |

As well as the 2 Measles, 1 Mumps, 1 Rubella and Meningitis vaccine or waiver requirements,
students enrolled in health care programs need to also submit a tuberculin
(PPD test with result) done within the past year, a tetanus immunization
within the last ten years and dates of a Hepatitis B series or a signed notice
of declination. Rubella immunity is required with no exception for age.
Students are also expected to have a complete physical, including immunizations,
by their health care providers prior to participation in their chosen program.
MCC students can receive MMR vaccination/vaccinations in Health Service
or from their private health care provider or from the Monroe County Health
Department. |

Where can
I find my previous records? |

Students can request
their high school immunization record, a military record or doctor's record
to determine vaccination history. |

We have moved several times and I can’t get my immunizations from my school. What should I do? |
| A |
If you have been to another college or in the military, you may request your medical records from them. You may also have a Blood Titer test for Measles, Mumps and Rubella taken by your doctor or a clinic. If the test comes back positive submit the lab results to Health Services and you will have met the requirements. If they are not positive, you will need to receive at least one MMR shot. |

If we have immunizations sent or faxed to Health Services do you call to tell me if you have or have not received them? |

No, we do not call individual students. If we receive your immunizations and you are not up-to-date we will send you a letter stating what you still need to have. |

My mom has my immunizations in my baby book, is that acceptable proof that I have received them? |
| A |
Unless the information is documented by a doctor, doctor’s office or nurse, we cannot accept it as proof of immunization. |
