General Student Population Immunization Requirements

Verification of Vaccination or Immunity

New York State requires proof of immunization against Measles, Mumps, and Rubella (German Measles) and either proof of meningitis vaccination or declination of vaccination to attend post-secondary educational institutions. This proof can consist of either:

  • 2 (two) MMR immunizations, OR
  • Positive serology for Measles, Mumps, and Rubella, OR
  • 2 (two) Measles immunizations given after your first birthday and 30 days apart AND
  • 1 (one) immunization each for Mumps and Rubella.
  • Meningitis immunization or signed waiver declining immunization

You may obtain proof of immunization from:

  • Your own physician, OR
  • High School or prior college records, OR
  • Military records

by sending this Request for Immunization Record form (.pdf format) along with MCC's Certificate of Immunization (.pdf format). If you have not had the required immunizations, you may obtain them from:

  • Your own physician (complete and return this form), OR
  • The Monroe County Health Department Immunization Clinic on Wednesdays from 12 noon to 6:30 pm. Located at 111 Westfall Rd. Please bring your Medicaid Card if you have one (complete and return this form), OR
  • MCC Health Services Monday to Friday from 8:45 am to 4:00 pm in Building 3- Room 165. You may be billed a fee. Your previous vaccine history must be on file in Health Services before you can receive a MMR immunization.

Meningitis Immunization

Certain college students are at increased risk for meningococcal disease, a potentially fatal bacterial infection commonly referred to as meningitis. In fact, freshmen living in dorms are found to have a sixfold increased risk for the disease. Read more about meningitis here.

The meningitis vaccine can be obtained from:

  • Your own physician (complete and return this form (.pdf format) ). OR
  • The Monroe County Health Department Immunization Clinic on Wednesdays from 12 noon to 6:30 pm. Located at 111 Westfall Rd. Please bring your Medicaid Card if you have one (complete and return this form), OR
  • You may decline to have the meningitis vaccine by signing and returning the Waiver portion of this form declining to receive the Meningococcal Vaccine.

Education and Health Promotion: Health Services works in collaboration with Residence Life to promote Health and Wellness in the Residence Halls. Through health programs and education, we assist students to maintain their health in order to succeed in their college experiences.

Nurses on Wheels (N.O.W.) is a program where the nurses from Health Services bring educational health programs to the Residence Halls.

Residence Life Health Requirements:

Because students in the residence halls live close together, the risk of acquiring and spreading disease is higher than in the general population. Due to this there are minimum preventative health standards required for living in the residence halls. One of these is the increased risk of acquiring meningococcal meningitis. Due to this risk, the meningitis vaccine is recommended for students living in residence halls.

  • Verification of MMR vaccination or titers proving immunity - same as general student population
  • Meningitis Vaccine - recommended, see above and click here for more information
  • Proof of Health Insurance - students who do not have health insurance may purchase MCC's Optional Sickness Insurance or purchase their own insurance plan
  • Illness/Injury Response: It is recommended that students living away from home be prepared to respond to emergency situations

Education and Health Promotion: Health Services oversees clinical rotation clearance of students in Health Career programs in accordance with New York State requirements. This clearance is required for the protection of patients at the clinical site and for the student’s protection from both infectious diseases and physical injury.

Health Career Student Health Requirements
Each of the programs below, with the exception of Health Information, are required to complete a Student Checklist Evaluation of Technical Standards that is designed to identify students who may be at risk of injury due to the demands of the program. The following programs are required to have all health documentation completed and cleared by Health Services before clinical rotations can begin:

These requirements are the same for all Health Career programs:

  • Physical exam by your Primary Care Physician (PCP) or a licensed clinic providing physical exam services. Physical exam must be submitted on the MCC Health Services Dept. Health History and Physical Form. (If you do not have a PCP, see below for resources.
  • PPD (Tuberculin skin test) within one year of the start of your program.
  • Tetanus booster within 10 years.
  • Verification of Hepatitis B immunization series, positive Hepatitis B titer, or signed Declination of Hepatitis B vaccination.
  • Verification of MMR vaccination or positive titers proving immunity.
  • Verification of Meningitis vaccination or declination.

Planning on being an MCC athlete? Here are a few things you need to know prior to participation on an athletic team at MCC:

In addition to the immunization requirements of all students in the general student population, athletes must also have:

  • PPD or Tuberculin skin test within the past 5 (five) years
    Tuberculosis is a communicable bacterial disease spread through the air when an affected person coughs or sneezes. Close contact during practice and travel to athletic events increase the risk of tuberculosis exposure for athletes.

    It is a requirement of the NJCAA that all students athletes have a negative tuberculosis screening test performed within the last 5 (five) years prior to sports participation.

  • Tetanus shot within the last 10 (ten) years
    Tetanus is commonly called lockjaw and is caused by toxins produced by a bacterial infection that is introduced to the body through a wound. This bacteria that produce the toxin are commonly found in the soil making athletes at increased risk of infection. These toxins attach to the central nervous system causing muscle spasms which can be fatal.

    Due to widespread immunization, tetanus has become a rare disease. For the protection of the health of student athletes, an updated tetanus shot is required by the NJCAA prior to participation on a sports team.

  • Sports clearance is to be completed prior to participation on an athletic team. Please contact Doug Henneberg, MS, ATC to schedule this clearance exam.

    Athletes are expected to submit a complete physical examination by their primary health care provider prior to the start of athletic participation. A pre-participation physical is targeted at identifying those students who may appear healthy, but have an underlying condition that athletic participation could worsen.

  • Student Athletes are required to have a comprehensive insurance plan while participating on an athletic team.
    Please refer to our information on Athletic Injury Insurance for more information. If you do not have your own insurance, you may purchase insurance from MCC for illness and/or injury. Please go to our Insurance page for complete information.
