"Do I need to complete the Employee Vax Verification form, or can I just email my vaccine card to HR?"

Employees need to complete the electronic Employee Vaccination Verification form if they choose to voluntarily submit proof of vaccination to the College.

In the last few weeks HR has received emails from several employees with their proof of vaccination attached. Thank you for vaccinating and for being proactive!  However, please note that HR is unable to upload your proof of vaccination to the electronic Employee Vaccination Verification Form.  Individual employees must complete the form and upload their proof of vaccination.

Here are instructions to access the Employee Vaccination Verification Form:

  • First, log into myMCC
  • Second, click on this link to the Faculty/Staff Resources & FAQs  
  • Third, click on the button at the top of the Faculty/Staff Resources & FAQs page that says "Employee Vaccine Verification"
  • Because you have already logged into myMCC, your M number, first and last name, and MCC email will already be filled in on the electronic Employee Vaccination Verification form.
  • Complete the remaining portions of the form and hit submit!
