Employee Vax Verification Form - add your booster info today!

The Employee Vax Verification Form has been updated to allow MCC employees to voluntarily submit booster shot information.

  1. You must be logged into myMCC to access the form. 
  2. Click the link above and select the form you would like to submit: Vaccination or Booster.
  3. Enter the information as it appears on your vaccine card.
  4. Upload a picture of your vaccine card.

Vaccine and booster data is collected via the Employee Vax Verification Form. Please do not email your vaccine and booster information to Human Resources.

Need help using the Employee Vax Verification Form?  We are here to help.  Human Resources will provide access to a desktop computer and scanner.  Human Resources staff members are available to assist you with logging in to myMCC, accessing the form, and scanning and uploading a copy of your vaccine card.
