New Program: Care Coordination - Community Health Navigation Certificate (CA01)

The College has received notification of the registration of a new program by the New York State Education Department for Spring 2021:

Care Coordination - Community Health Navigation Certificate (HEGIS Code 5201)

This certificate program will prepare students to enter the workforce as Care Coordination Community Health Navigators or continue their credentialing in a health-related field.

Students will develop critical skills allowing them to become advocates for the population for which they serve. Students will gain an understanding of community health and the healthcare system. They will learn skills in health information technology as well as specific communication and motivational interviewing techniques to provide guidance and support of patient behaviors that promote value based outcomes via reduction in chronic disease, hospital admissions, and healthcare costs. Within this certificate program, students will also have the opportunity to earn certification in Health and Wellness Coaching.
