STUDENT EVALUATION OF FACULTY SURVEY DISTRIBUTION

In accordance with the Student Evaluation of Faculty Policy, the Student
Evaluation of Faculty Questionnaire may be administered anytime, at the
discretion of the departments/faculty, between October 31st and the last day of
classes for the Fall semester. The Questionnaire should be used by new
full-time and new part-time and adjunct faculty across the College, and, this
year, by all full-time, part-time and adjunct faculty in the divisions of
Science, Health and Business and Interdisciplinary Programs.

For the Brighton Campus:

The Questionnaire materials and pencils will be available in your Department
Office after October 14th. A box will be placed in your Department's main
office, and you can pick up your evaluation forms from that box or from your
mail folder during your Department's regular office hours. If you cannot pick
up the materials during regular office hours and you do not have a key, please
call your adjunct coordinator or your Department Chair or secretary to make
alternative arrangements.

For the Damon City Campus:

The Questionnaire materials and #2 pencils will be available between the hours
of 8 a.m. and 4 p.m. in Room 4225, office of Linda Byer, Faculty Secretary.
Materials for Evening and Saturday classes can be picked up in Room 4258, the
Integrated Learning Center, evenings between 4 p.m. and 7 p.m. and on Saturday
between 9 a.m. and 2 p.m.

For Off-Campus Faculty who do not teach any courses at Brighton or Damon:

Contact your Department's Secretary or Adjunct Coordinator to make arrangements
to pick up and drop off your materials.

Parts I and II of the Questionnaire must be returned no later than the last day
of classes, December 13, 2002. Instructions for the administration and return
of the questionnaire are in each packet.
