Student Opinion Questionnaire Reminder


All teaching faculty will be receiving their Student Opinions of Course and Faculty Questionnaires in the next week. I want to take this opportunity to remind all faculty that you should use a student proctor to administer and return these questionnaires.  Each packet included student proctor instructions.  We have had numerous instances of faculty members returning their own surveys to our office.  To ensure validity of the results we ask that you follow the instructions in your packets.

Ask a student representative to distribute the materials. Request that the student representative collect the materials at the end of the time you have designated for the Questionnaire.  Ask the student to:

1)   Place all surveys into the inter-office envelope marked “Return to Academic Services”
2)   Return the completed forms to the following locations.

Brighton Campus:          Campus Mail – the main receptacle can be found in Building 1, first floor outside of Room 1-103.

Damon City Campus:     Anne Kirkpatrick, Faculty Secretary, Room 4225, 8:00am – 4:00pm The Integrated Learning Center, Room 4258, 4:00 pm - 6:00 pm and on Saturday between 9 a.m. and 1 p.m. If both offices are closed, instruct the student to place the envelopes under the door of Room 4225. 

Applied Technology:       Main Office, Room 158.

PSTF:                               Box outside Room 216M

Extension Sites:              

East Rochester - Room 520 – A-V Equipment Room
Greece - Terry Rood, Continuing Ed. Office
Webster - Continuing Ed. Office Room #N-16
Spencerport-  Sue Mosher, c/o High School Main Office

The Questionnaires must be returned no later than the last day of classes, December 12, 2008. 

In accordance with the Faculty Senate Policy, the Student Opinion Questionnaire may be administered anytime, at the discretion of the departments/faculty, between now and the last day of classes for the Fall semester.  It is recommended that the questionnaire not be administered within a two-day period of a unit or final exam.

Any narrative form of survey must be designed, conducted and collected through departmental processes and will not be connected to the college-wide instrument.  The surveys will be processed during early January.  If you have any questions regarding the procedure, please call Sue Smith, Extension 2195.  We thank you for your assistance in this important project.
