Recent Excellus Mailing Requesting Social Security Numbers


MCC’s healthcare insurance carrier, Excellus, recently sent a mailing to some subscribers whose dependents have missing or inaccurate Social Security numbers in Excellus’ system.  Excellus is requesting this information in an effort to comply with the record keeping and reporting requirement of the Affordable Care Act.  Insurers and employers will be gathering this information to comply with Internal Revenue Service requirement that they show proof of healthcare coverage for you and your dependents. 

This information was not previously required for databases upon enrollment in a healthcare plan; as a result, you are being asked to furnish the information at this time.

There are several methods of providing this information:

1.    You may complete the form as instructed for Excellus;

2.    You may call the Excellus Customer Service line using the 800 number on the back of your Excellus card; or

3.    You may log into Excellus’ secured website at: excellusbcbs.com/ssn. 

Please send a copy of the completed Excellus form to the Human Resources Office.  Failure to provide the requested Social Security number(s) may require you to report this information to the IRS at some later date.
