We would like to remind you that all Flexible Spending Account (FSA) forms must be received by the Human Resources Department by November 30, 2007. We will be unable to accept forms after that date.
2008 403(b) Tax Deferred Annuity Forms
You must complete a new 403(b) Salary Reduction Agreement (SRA) if you are electing a new SRA limit for 2008. Your goal amount from 2007 will roll into 2008, up to the general limit ($15,500), unless you complete a new form.
Any forms received by the Human Resources Department by November 30, 2007 will be processed on the first pay date in the new year (January 10, 2008); forms received after November 30th but before January 10, 2008 will be process on the January 24th pay date.
