MCC Employees: Voluntary Vaccine Verification Form is now Live

SUNY and MCC encourage all students and employees to get vaccinated for COVID-19 as part of our comprehensive efforts to promote health and safety. A key component of the MCC Summer/Fall Semester 2021 Return Plan is voluntary vaccine verification by employees.

Frequently Asked Questions:

  1. What information do I need to complete the Employee Vax Verification?  The Employee Vax Verification form is electronic.  You will need to fill in the type of vaccine you received and the dates you received it.  The necessary information can be found on your vaccine card. You will need to submit a screenshot or PDF copy of your vaccine card or NYS Excelsior Pass proof of vaccination.
     
  2. How long does it take to complete the form? 1-2 minutes.  Once complete, you will receive an email confirmation that your form has been submitted.  An HR staff member will review your submission and you will receive a second email confirming that your submission has been approved.
     
  3. Is MCC mandating the vaccine for employees at this time? No. MCC is encouraging employees to get vaccinated, however vaccination is not required to work in person.
     
  4. How will the privacy of my vaccine-related information be maintained? The ADA requires MCC to maintain the confidentiality of employee medical information, such as documentation or other confirmation of COVID-19 vaccination. This information, like all medical information, must be kept confidential and stored separately from the employee’s personnel files under the ADA. The College will report only aggregate data with respect to vaccination status – e.g., As of X date, X number of employees voluntarily reported that they are fully vaccinated.
     
  5. Why is MCC asking for employees to voluntarily verify their vaccine status? As of August 2, 2021, most health-related restrictions will be eliminated for all "fully vaccinated" students, faculty and staff. Individuals are considered "fully vaccinated:" (1) two weeks after receiving the second dose in a two-dose COVID-19 series or (2) two weeks after receiving a single dose vaccine. Employees who submit their vaccine verification form are not required to participate in weekly COVID-19 testing and are not required to wear a mask, except in instructional spaces such as classes, labs and rooms with an assigned course section number.
     
  6. What if I am not vaccinated or what if I choose not to report my vaccination status to MCC? Unvaccinated individuals and individuals who choose not to verify their vaccine status must follow the latest U.S. Center for Disease Control and Prevention (CDC) guidance, including wearing a mask, social distancing and participating in weekly COVID-19 testing.
