ACA 1095C Tax Form

Full-time benefit eligible employees will receive their 1095C form from Paragon Compliance LLC  via U.S. Mail within the next week .  Under the Affordable Care Act, employers are required to provide Form 1095C to employees that were full-time and benefit eligible during the 2023 calendar year.  This form includes information regarding the health insurance coverage offered by the employer and whether or not the employee enrolled in coverage. 

New for MCC 1095C forms in 2023 - Self-funded health care plans are required to report eligible dependent(s) covered in the plan year.  Anyone who covered a spouse and/or children on their medical policy in 2023 will see them listed on the form.
