Dynamic Forms URL Update

Recently we updated Dynamic Forms for improved performance with our single sign on and multi-factor authentication. Due to that, the URL for internal forms has changed.

If you have any saved URL’s that are used for email distribution or for marketing purposes, please submit a Technology Services Support Request Ticket to obtain a new URL and an embedded URL for distribution. This only affects internal use dynamic forms. The URL for external forms did not change. 

All of the forms on the web have been updated already along with any associated Go Links.

The old URL’s will be shut down at the end of May. Please note - we need to supply you with these links as you cannot copy the URL in your browser as that is specific to your sign on and will cause the user to see an error code.
