As a valued employee of Monroe Community College, you are being given a special opportunity to purchase Supplemental Term Life Insurance coverage for yourself whether or not you are currently enrolled in Basic Life (2 ½ times Annual Salary) . The College is adding a new option of Supplemental Term Life at 1 times your salary in addition to the 1 ½ times your salary that is currently offered. The Supplemental Term Life is being offered at affordable group rates based on your age with a guaranteed issue of $100,000. Supplemental Term Life that you elect in this enrollment period will be effective on September 1, 2009.
This opportunity to purchase Supplemental Term Life coverage, without medical questions being asked, and with or without being enrolled in the Basic Life, is being offered for a limited time only. If you do not elect supplemental insurance within this enrollment period (the month of May), you will be required to wait for the next annual enrollment, will be required to enroll in the Basic Life first, and will be required to provide evidence of good health at that time.
You may purchase this insurance at group rates through the convenience of payroll deductions. Should you purchase insurance; deductions will begin on September 3, 2009.
