Open Enrollment begins Tuesday, November 1, 2011 and continues through the month of November.
Excellus Representatives and Human Resources staff will be available to answer questions about health care plans or assist you in completing enrollment forms on the following dates and times:
Brighton Campus:
Monday, October 31st from 9:00 to noon in the Empire Room, Warshof Conference Center, Flynn Campus Center
Damon Campus:
Thursday, November 3rd from 9:00 to noon in the DCC Board Room
Health care enrollment forms, plan comparison documents and employee contribution rate sheets are attached. Please note that contribution rates for Blue Point 2 Enhanced and Blue PPO have increased for 2012, in some cases significantly.
If you are not making a change to your medical or dental plans, no action is necessary on your part.
If you are making a change to your plan please return your form to the HR Office as soon as possible and no later than November 30, 2011. You may also leave completed enrollment forms with a Human Resources representative through November.
