Open Enrollment begins Monday, November 1, 2010
Excellus Representatives and Human Resources staff will be available to answer questions about health care plans or assist you in completing enrollment forms on the following dates and times:
Brighton Campus Tuesday, November 2 9:00 a.m. – 12:00 noon Warshof Conference Center, Flynn Campus Center (Empire Room)
Damon Campus Thursday, November 4 9:00 a.m. – 12:00 noon Room 4172
Health care enrollment forms, plan comparison documents and employee contribution rate sheets are attached. Please note that contribution rates for Blue Point 2 Enhanced have increased for 2011. You may decide to make a change based on the 2011 rates.
If you are not making a change to your medical or dental plans, no action is necessary on your part.
If you are making a change to your medical or dental plans please return your form as soon as possible and no later than November 30, 2010. You can leave completed enrollment forms with a Human Resources representative on the above dates or you can drop them off or send them to the Human Resources Office.
