Open Enrollment begins October 28, 2009.
Excellus representatives and Human Resources staff will be available to answer questions about health care plans or assist you in completing enrollment forms on the following dates and times:
Brighton Campus Wednesday - October 28th 9:00 am – 12:00 noon Empire Room
Damon City Campus Thursday - October 29th 9:00 am – 12:00 noon Conference Room 5057
Health care enrollment forms, plan comparison documents and employee contribution rate sheets are attached. You may decide to make a plan change based on the 2010 rates.
If you are not making a change to your medical or dental plans, no action is necessary on your part.
If you are making a change to your medical or dental plans you are encouraged to return your enrollment forms as soon as possible and no later than November 30, 2009. You can leave completed enrollment forms with a Human Resources representative on the above dates or you can drop them off or send them to the Human Resources Office.
