Brighton Campus 10:00 p.m – 11:00 pm Empire Room (B & C Shift Employees)
Excellus representatives and Human Resources staff will be available to answer questions about health care plans or assist you in completing enrollment forms.
Every employee who has health care insurance through MCC must complete a new enrollment form. Health care enrollment form and employee contribution rate sheets are attached.
You are encouraged to return your enrollment forms as soon as possible. You can drop your form off today in the Empire Room and at the Damon Campus Conference Room 5057 or as soon as possible in the Human Resources Office at the Brighton Campus.
Special Note:
On November 9, 2007, from 9:00 – 4:00 pm, Human Resources staff will be available in the HR Conference Room to further assist you with answers to questions and to receive enrollment forms.
It is imperative that forms are returned to the Human Resources Office as soon as possible as the HR staff is operating on a strict processing deadline.
