The Controller’s Office has updated the travel forms online. You may access them through the “MyMCC” portal by selecting the “Employee” tab, then scroll to the “Controller’s Office” within the “Online Forms and Documents” channel.
Samples of the “Prior Approval Request Form”, “Travel & Conference Expense Form”, “Local Mileage & Conference Expense Report”, and “Blanket Prior Request Approval Form” are attached for your information.
