Dynamic Forms Issues

On Wednesday, the vendor, Dynamic Forms (used by many departments), pushed live an update that is causing issues. Known issues are:

  • uploads not being able to be viewed
  • data columns not lining up with the correct data
  • data showing as numbers/symbols instead of letters
  • return for revision being 'stuck'
  • drafts not able to be accessed
  • dropdown fields not displaying the options

Please be aware that your form submissions and workflows might not act correctly. 

This is a known vendor issue that is system-wide and the vendor is working to resolve all issues. 

An update will be provided when the vendor resolves all issues.
