NYS Dept. of Labor Record of Employment form

Per the NYS Department of Labor, employers are now required to provide a Record of Employment form to employees whose employment has terminated.  The College has automated the process of identifying part-time, temporary and adjunct staff members whose current assignment with the College will be ending in the near future.  Forms will be sent to staff via email; in cases of no available email address, forms will be sent via U.S. Mail.  This process will be ongoing and will begin the week of May 13, 2024.

Employees receiving the Record of Employment form may be eligible for an unemployment benefit. For more information, use this form and contact the NYS Unemployment Division regarding eligibility and benefits.
