Improve health and get a chance to win a $500 gift card

MCC students are needed to take a survey that will help shape the region's health priorities. Please take the survey by Sept. 3 at MyHealthStory2018.com or MiHistoriadeSalud2018.com and share the link with friends.

All answers are confidential, and survey participants can enter a chance to win a $500 gift card. The survey is conducted by Common Ground Health, the regional health planning organization, in partnership with county health departments.
