Put together a rewarding career in a rapidly growing health care field with MCC’s Care Coordination ̶ Community Health Navigation Certificate. Through this program—just one of two in New York state—you’ll learn to take a leading role in educating patients and connecting them to essential health care and support. Care Coordinators work with doctors, nurses, physical therapists, mental health counselors, and other health care professionals. They are essential to helping patients improve quality of life, reduce disease and minimize hospitalizations.
This Certificate aligns with MCC’s AS Health Studies program. Students in the program can use their electives to complete the certificate alongside their degree. Career changers and health care professionals are also eligible to take the certificate and build new credentials.
Through lectures, coursework and discussions, you’ll learn the critical skills you need to:
- Advocate for whatever populations you serve
- Gain an understanding of community health and the healthcare system
- Use health information technology effectively
- Develop communication and motivational interviewing techniques that guide and support patient behaviors
- Help control chronic disease, hospital admissions and healthcare costs
Upon completion, you’ll be eligible to pursue industry certification in Health and Wellness Coaching. Care Coordinators support a wide range of health care providers, including hospitals, community health centers, sports and physical rehabilitation centers, senior living centers, nursing homes and surgical centers. Employment Outlook
There is a growing need for care coordinators with both clinical and non-clinical experience and knowledge of community-based resources. Candidates who speak more than one language are also in demand. Employment of health educators and community health workers is projected to grow 11 percent from 2018 to 2028. This is much faster than the average for all occupations, according to the U.S. Bureau of Labor Statistics.

Description
This certificate program prepares students to enter the workforce as Community Health Navigators or to continue their credentialing in a health-related field in Care Coordination. Students will develop critical skills to become advocates for the population(s) they serve and support. Students will gain an understanding of community health and the healthcare system. They will learn skills in health information technology, as well as specific communication and motivational interviewing techniques. Students will gain knowledge of how to use these skills to provide guidance to patients and support patient behaviors that promote value-based outcomes via reduction in chronic disease, hospital admissions, and healthcare costs. When implemented by Care Coordinators or Community Health Navigators, skills learned as part of this certificate will facilitate more efficient use of practitioner time, creating cost savings within the healthcare field. Within this certificate program, students can earn the Multicultural Diversity World Health Perspective microcredential and utilize coursework toward eligibility for certification in Health and Wellness Coaching. Courses in this Certificate can be applied toward course requirements in the MCC Health Studies AS degree program.
