Records Request
All requests for access to public records must be submitted in writing. Requesters are encouraged to use MCC’s online Application for Access to Records form, and to describe the requested documents as specifically as possible.
Inquiries using the online form will require a Dynamic Forms account:
- MCC employees and students can access their account using their Microsoft credentials.
Application for Access to Records form for MCC Employees & Students - Members of the general public will be required to create an account the first time they access Dynamic Forms. Please refer to our instructions on creating an account for more information.
Application for Access to Records form for Community Members
MCC will respond, via email, within five business days of receipt of a request by:
- granting or denying access to records, in whole or in part;
- acknowledging receipt of the request, including an approximate date when the request will be acted upon; or
- informing the requester that the documents sought are not reasonably described, and clarification is needed.
