Immunization Requirements

New York State Public Health Law requires all college students enrolled for 6 credits or more to submit (1) proof of Measles, Mumps, and Rubella (MMR) and (2) to review information about the Meningococcal vaccination to make an informed decision. Students who do not meet both of these requirements by September 23th, 2025 will be withdrawn from all in-person/hybrid courses and a registration hold will be placed on your account. Please refer to the Step-by-Step Instructions for Uploading Immunization Information into the portal in order to fulfill each requirement.  

MMR Immunization Requirement

To comply with this requirement, students must submit proof of one of the following through the myWellness Portal:

  • Two doses of MMR vaccine given after your first birthday
  • Two doses of Measles and one does of Mumps and one dose of Rubella vaccine (given after your first birthday)
  • Medical documentation of history of disease(s)
  • Blood titers showing immunity (lab report required)
  • Born before 1/1/1957

Meningococcal ACWY or B (Meningitis) Immunization Requirement

Refer to the meningococcal fact sheet and then make an informed decision either to decline this vaccine or to show proof of vaccine(s):

To decline this immunization, login to the portal to complete the required form:

  1. Go to the Immunization Requirements tab
  2. Scroll to the bottom of the page
  3. Click on Clearance Exemptions
  4. Select Meningococcal ACWY or B
  5. Complete the waiver; and
  6. Upload to the portal

Please note: Declining this vaccine ensures compliance with NYS Public Health Law but does not prohibit you from getting the vaccine at a later date. Students may elect to receive the vaccine at any time in the future through their healthcare provider, the Monroe County Department of Health Immunization clinic or at a local pharmacy.

To show proof, login to the portal to enter dates and show proof of these vaccines:

  • Meningococcal ACWY (i.e., Menactra, Menveo or MenQuadfi) vaccine within the last five years or
  • Meningococcal B (i.e., Bexsero or Trumenba), two dose series at any time.

Please note: Look carefully at the dates of these vaccines because the most common reason students remain in "non-compliance" after submitting records is because their vaccine is over five years old &/or they only have had one Meningococcal B vaccine.
