Student Athletes

MCC follows the National Junior College Athletic Association requirements for all athletes. Both new and returning athletes must submit proof of a sports physical each year, complete their student athlete section and upload all documentation to the Student Health Services Portal once complete. 

How to Submit Your Required Documentation


MMR (often given as 2 MMR immunizations in childhood)
2 Measles immunizations
1 Mumps immunization
1 Rubella immunization
or
Blood test (known as a blood titer) for measles, mumps and rubella showing immunity


Meningitis ACWY vaccine within the last five (5) years (may appear on your records as Menactra, Menveo or MenQuadfi)
or
Meningitis B, two or three dose series at any time (may appear on your records as Bexsero or Trumenba)
or
Decline the meningitis immunizations at this time.  There is no penalty for declining these immunizations.  Completing the waiver form with the student's signature and date ensures compliance with NYS Public Health Law.  Students may receive the vaccine at any time in the future through their healthcare provider or at a local pharmacy.
To decline the meningitis immunization, visit the Student Health Services Portal.  Go to the Medical Clearances tab.  Scroll to the bottom of the page, click clearance exemptions and select Meningococcal ACWY or B.  Complete the waiver and upload back to the portal.


Tdap (Tetanus, Diptheria, Pertusis) vaccine received within the last ten years. 


Annual Athlete Physical Exam
Athletes must submit a physical exam completed within one calendar prior to their start of athletic participation at MCC.  If the athlete's physical is not completed on the MCC physical exam form found in the Student Health Services Portal, the exam submitted must indicate the athlete was cleared for athletic participation.  This physical is due on an annual basis for as long as a student is participating in MCC athletics. 


Proof of Health Insurance Information
Students are required to submit proof of current health insurance in the Student Health Services Portal.
