Description
This certificate program will prepare students to enter the workforce as Care Coordination Community Health Navigators or continue their credentialing in a health-related field.
Students will develop critical skills allowing them to become advocates for the population for which they serve. Students will gain an understanding of community health and the healthcare system. They will learn skills in health information technology as well as specific communication and motivational interviewing techniques to provide guidance and support of patient behaviors that promote value based outcomes via reduction in chronic disease, hospital admissions, and healthcare costs. Within this certificate program, students will also have the opportunity to become eligible for certification in Health and Wellness Coaching. Program Learning Outcomes
1. Describe select chronic diseases frequently encountered by healthcare navigators.
2. Explain how to access the community services available to their clients.
3. Demonstrate health coaching skills to successfully educate, motivate and empower patients/clients.
4. Accurately document patient/client needs to support team-based, patient-centered care.
5. Demonstrate cultural sensitivity in client interactions.
