MCC Registration Form — If you’re registering in person or by mail.
Personal Information Change Form — To update or correct your personal information on file at MCC.
Certificate of Residence Form — This documents your permanent address. You must submit either a certificate of residency OR residency affidavit to MCC once each year.

SUNY Cross-Registration Online Form — For full-time MCC students who want to register for a class at a SUNY school.
RAC Cross-Registration Enrollment Form — For full-time MCC students who want to register for a class at a Rochester Area College (RAC program).
