How to Request Accommodations
Step 1: Submit Disability Documentation
- Documentation can be submitted via email, fax, or in-person.
- Documentation must be written by a licensed or credentialed examiner. Examples of documentation include a Full Psychological Evaluation/Diagnostic Report, Individualized Education Plan (IEP), 504 Plan, Audiogram, Speech Language Evaluation, and/ or Medical Letter/Report (on letterhead).
- For additional information, please refer to the General Guidelines for Documentation.
- Due to the interactive nature of any request for accommodation, students are encouraged to provide as much relevant documentation to help support all requests for services.
- Documentation is reviewed in the order received. Therefore, all requests should be made at least 30 days in advance of the need.
Note: The Disability Services office is the designated office for determining accommodations for the Accuplacer (Placement Test).
Step 2:  Schedule an Appointment with a Disability Services Counselor
Once documentation is reviewed an email will be sent to the MCC student email account outlining the next steps in the process to request accommodations.
Students are encouraged to follow up with the office if they have not received any correspondence via email or phone after four weeks of initial submission of documentation.
Type of appointment to be scheduled:
- An Intake Meeting is required for any new student requesting accommodations.
- A Reinstatement Meeting is required for any returning student who takes the fall/spring semester requesting to reactivate accommodations.
- A Modification Meeting is for a student currently registered with Disability Services requesting to add/remove accommodations.
Step 3: Attend Required Meeting
An interactive meeting occurs between the student and Disability Counselor to develop an Academic Accommodation Letter. Each meeting includes the following topics:
- Request for accommodation(s),
- Disability documentation submitted,
- Identify the limitations/barriers,
- Review policies and procedures,
- Complete required paperwork,
- Review available supports/resources external to the department
At the conclusion of the meeting, the student is provided with necessary information including, but not limited to their Academic Accommodation Letter, importance of communication, supports, etc.
