Temporary Accommodations
Students who experience changes in their condition or temporary disabilities may require accommodations to ensure continued access to programs and activities. Temporary accommodations are determined on a case-by-case basis due to injury, surgery, short-term medical conditions, concussions, pregnancy-related complications, or similar circumstances. Assistance cannot be provided for tasks of a personal nature such as scribing for homework, individual aid assistance, or assisting with personal health-home care.
Step 1: Log into the MyWellness portal, select the "Request Accommodations" tab, and complete the Academic Accommodation Request Form.
- Documentation must be written by a licensed or credentialed examiner. Examples of documentation include a Full Psychological Evaluation/Diagnostic Report, Individualized Education Plan (IEP), 504 Plan, Audiogram, Speech Language Evaluation, and/ or Medical Letter/Report (on letterhead).
- Due to the interactive nature of any request for accommodation, students are encouraged to provide as much relevant information and documentation to help support all requests for services.
- Documentation is reviewed in the order received. Therefore, all requests should be made with as much advanced notice as possible.
Step 2: After each initial Academic Accommodation Request Form has been reviewed the student will receive a message from Disability Services in their MyWellness portal with instructions to schedule an Intake Appointment.
Students are encouraged to contact Disability Services if they have not received any correspondence via the myWellness portal or by phone after three weeks from the initial submission of the request.
Step 3: During an Intake Appointment the student engages in an interactive process with a disability counselor regarding the Academic Accommodation Request Form, supporting documentation, barriers experienced in the academic setting, requested accommodations, completion of necessary paperwork, policies/procedures, accommodation letters, available support services, and any other information that is relevant to the student. Temporary accommodations become active once the student delivers their Academic Accommodation Letter to each instructor.
